Fill out the label fields. To enter choices and other information, click on the gray settings cog symbol at the far right. This will open further options for creating the question.

To change the order of the questions, click and hold down on the three bars symbol to the left of the label field. Then drag the question up or down to the new survey location.

To delete a question field, click on the red "x" symbol at the far right of the question.
If a question is deleted, it is not possible to undo this action.

To add another question field, click on the gray "Add another field" button. A new question field will appear.

You can add carbon accounting fields for collecting scope 3 category emissions data from partners, suppliers or stakeholders.
Click on the gray "Add carbon accounting field" button. The question fields will appear. To enter choices and other information, click on the gray settings cog symbol at the far right. This will open further options for creating the question.
Below is a description of the carbon accounting question fields.

Label | Yes | This is the information or question you want to receive from or ask of the respondent. |
Scope 3 allocation category | Yes | Select the scope 3 allocation for which you want to collect data. See full list here. |
Do you want to average the emissions from this field across the entire year? | Yes - switch this option "on" or "off" | If you want to average the emissions for this scope 3 category across the entire year, makes sure to switch this field on. |
Allocation month and year | Yes | If the "average across the entire year" field above is switched on, select the year. If the "average across the entire year" field above is switched off, select the month and year. This means you want to collect specific data for that month and year. |
Placeholder text | No | You can enter text as placeholder text that will show up in gray as example text for the question. |
Help text | No | You can enter help text to provide more information, description and context to the question. |

You can link custom metrics and KPIs to your surveys.
First create a custom KPI.
Enable this feature on and select the custom KPI from the dropdown menu.
A new label or question will appear in your survey form that is linked to that KPI.
When a respondent fills out the survey, the data will be saved to that custom KPI.
Note, you must add the custom KPI to your ESG Dashboard, a goal, action or asset in order to be able to see the data outside of the survey.

You can create custom fields for surveys if you want to continuously keep a record of specific data for a partner or supplier.
When you select the custom field, the data will be stored in your partner's record.
To create a custom field, navigate to your account Setting and Manage Custom Fields.
Ensure the field is switched on. Then select the custom field from the dropdown menu.
Once the you select the custom field, a new label or question linked to that field will appear in your survey form.
When a respondent fills out the survey, the data will be saved to that custom field record for that partner or supplier in your Brightest partner directory.

You can link a Brightest action to this form if you want viewers of that action to be redirected to a form.
First create an action time in your Action Module.
Then switch on the option "Do you want to link an action to this form?"
Select the action you want to link from the dropdown menu.
When an action is linked to your form, this means when someone signs up for your action they will be redirected to your form (example: you want new signups to fill out an application)
